Who qualifies

Is Family Model an option for us?

Family Model Residential Support is for adults with intellectual or developmental disabilities who get services through Tennessee’s home- and community-based programs.

  • Enrolled in one of the programs we serve: a Tennessee DDA 1915(c) waiver or Employment and Community First (ECF) CHOICES
  • Family Model is chosen by the person and included in their person-centered plan (ISP)
  • Lives in East Tennessee, or wants to move there. Ask us about your county.
Not enrolled yet? Contact DDA or your support coordinator about eligibility. You can still call us with questions.

Family questions

What if my loved one isn’t enrolled in a waiver yet?

Start by contacting DDA (the Tennessee Department of Disability and Aging) or a support coordinator about enrollment. Call us too. We’re glad to help you understand the next step.

Can I be the paid caregiver for my family member?

No. A mother, father, grandparent, sister, brother, son, daughter, or spouse — by blood, marriage, or adoption — can’t be paid to provide Family Model services.

Can we visit before deciding?

Yes. Visiting a home is part of deciding. You, your loved one, and the caregiver all get a chance to meet before anyone moves. The person chooses where to live.
